  • Student Declaration form
  • Certified true copies of:
    • Birth Certificate
    • GCE (A/L) Certificate
    • GCE (O/L) Certificate
    • NIC
  • Pay-in slip (payment of admission/registration fee of Rs. 3350/=)
  • 3 copies of 45.0×50.0 mm (passport size) photographs
  • Affidavit (if there are changes of the name other than the birth certificate)
