Please note that officers from the examination division/UOC will visit the faculty of Science to collect the hard copies of documents listed below, which are needed to complete your personal files (you have already submitted soft copies online)
– Student Declaration from
– Certified true copies of
     Birth Certificate
     GCE (A/L) Certificate
     GCE (O/L) Certificate
     NIC
– Pay-in slip ( payment of admission of registration fee of Rs.3350/=
– 3 copies of 45.0×50.0 mm(passport size) photographs
– Affidavit (if there are changes of the name other than the birth certificate)
