This is to bring to your attention that the Supplication Forms for the General Convocation 2019 can be accessed via https://sis.cmb.ac.lk/sci/my_profile/supplication_form

Students are also reminded to indicate their names correctly since no changes will be made to the Degree Certificate when its printed.

The deadline to submit the Supplication Form is 28/07/2020 and the submissions made after the deadline will not be accommodated.
